Pharmacist in DSFH Madinah/Saudi Arabia, Madinah
Duties and Responsibilities:
- Reviews, interprets, clarifies Doctors medications orders and Verifies ALL orders doses, duration, and checks for potential drug to drug interaction and clinical indication. Communicates with ordering doctor in case of any ambiguity or concern
- Oversees the preparation of the verified medications, performs double-checking and review of proper labeling for the prepared medications and then dispenses.
- Calculates the amount of the drug product to dispense for a prescription based upon body weight and patient age.
- Performs computer entry of outside prescriptions or orders into CPOE of Fakeeh care HIS before dispensing .
- When covering outpatient areas, Ensures prompt and timely calling of patients to dispensing windows and aims at reducing patients waiting time to the minimum possible
- When covering inpatient pharmacies, responsible for tracking on the whiteboard all patients that have discharge order and immediately preparing take home medications and refunding / arranging for return of dispensed and non-administered medication.
- Counsels the patients and caregivers on the proper & safe use of medication, storage, and interactions, etc.
- Follows all medication safety procedures when dispensing medications
- Maintains control & security of all drugs including dispensing, record keeping and secured storage of narcotics and controlled medications.
- Identifies and appropriately handles the nearly expired medications according to hospital policies and NEVER dispenses a medication beyond its expiry date.
- Participate in chemotherapy preparations program(order verification ,protocol checking ,…etc)
- Participate in the intravenous admixture preparations program, mixing and checking including total parenteral nutrition.
- Properly completes charging of patients for dispensed medications.
- Demonstrates knowledge of proper storage & handling of hazardous materials found in her/him workplace, as well as first aid measures necessary for accidental exposure.
- Demonstrates knowledge & skills in utilizing formulary, identifications of generic substitution, indication of the role of deductible limits and application of prescription quantity limitations.
- Practices and fully complies with current infection prevention and control standards when handling medications and preparing IV AND TPN formulas
- Contributes to training and induction orientation for newly hired pharmacists and students and trainees.
- Demonstrates knowledge of commonly used over the counter (OTC) drugs.
- Reports to the Senior pharmacists and enters on OVR any encountered medication error or ADR.
- Attends continuing education programs to maintain expertise in Pharmacy and required licensure.
- Always complies with hospital and departmental policies and procedures.
- Ensures Enrollment in continuous education program regarding medication management practices , high risk medications, medication errors and others.
- Ensure the staff training and implement all the CIBAHI & JCI requerment to be in place by providing a trace activities with a time frame and action plan adding to that he/she shall report to the pharmacyutical services director a monthly report with the progress
- Ensures that all plans, policies and procedures, protocols, clinical practice guidelines and scope of services for the pharmacutical services are up-to- date and uploaded and communicated to the quality team to upload to the system.
JOB REQUIREMENTS / QUALIFICATIONS:
Bachelor’s Degree of Pharmacy in the minimum requirement.
One year experience in hospital pharmacy setting is recommended.
Good command of oral and written Arabic and English communication
Must have and maintain a current Saudi and country of origin pharmacy license.
