The Clinical Pharmacist plays a pivotal role in delivering high-quality pharmaceutical care within a tertiary hospital. This position involves optimizing medication therapy, ensuring the safe and effective use of medications, and collaborating with a multidisciplinary team to enhance patient outcomes. The Clinical Pharmacist will also contribute to education, research, and quality improvement initiatives

Medication Management:

  • Evaluate and interpret prescriptions to ensure appropriate medication therapy.
  • Dispense medications and provide detailed instructions to patients.
  • Monitor patient health and progress to ensure medications are effective and adjust dosages as necessary.
  • Identify potential drug interactions or adverse effects and take appropriate steps to prevent or manage them.

Patient Care:

  • Conduct clinical medication reviews and provide recommendations for therapy adjustments.
  • Participate in patient care rounds and collaborate with physicians, nurses, and other healthcare professionals.
  • Provide medication-related counseling and education to patients and their families.
  • Assess patient needs and incorporate age and disease-specific characteristics into drug therapy and patient education.

Clinical Interventions:

  • Provide clinical interventions to detect, mitigate, and prevent medication adverse events.
  • Conduct research and practice advancement projects to improve medication-use processes.
  • Participate in quality improvement initiatives and ensure regulatory compliance.

Education and Training:

  • Serve as a departmental resource and liaison to other departments and hospital personnel.
  • Provide medication and practice-related education/training to healthcare staff.
  • Actively serve as a preceptor for pharmacy students and residents.

Operational Support:

  • Support centralized and decentralized medication-use systems.
  • Track and evaluate pharmacy programs for operational, quality, and financial efficiency.
  • Design and implement stewardship activities and restriction/surveillance programs.
  • Doctor of Pharmacy (Pharm.D.) degree from an accredited institution.
  • Current pharmacist license in the state of practice.
  • Completion of a pharmacy residency program (PGY1 and/or PGY2) preferred.
  • Strong understanding of pharmacology and medication therapy management.
  • Excellent communication and interpersonal skills.
  • Ability to work collaboratively in a multidisciplinary team.
  • Minimum of 2 years of clinical pharmacy experience in a hospital setting.
  • Experience with electronic health records (EHR) and pharmacy information systems.
